Cases: May 08 53-yrs old male complains of prurigo, redness and scaling for 6 month localized to the right hand.
No premorbid skin diseases besides malformation of the 3.th right fingernail.
Treated with topical steroids for eczema without effect.
What diagnosis most be suspected and what treatment should be considered?

Dermatophytosis (trichophyton rubrum) most propably derived from the affected nail.
Unilateral affection supports the diagnose which can be confirmed by microscopic and culture investigation. Use of topical steroids reduces the inflammatory stigmata but maintain the existens of hyphae.
Treatment with Tbl. Terbinafin 250 mg daily for 2 weeks is usually curative.
